Our system is set up to securely provide your full medical record and does not allow us to select or provide only specific documents, consultations or sections of your record.

If you only require specific information or documents from your medical record, you may still make this request; however, you will need to review the full record provided and identify the information you require yourself.

By submitting this form, you acknowledge and understand that your records will be provided in full and that we are unable to select specific parts of the record on your behalf.

Proof of Identity

To help us verify your identity and locate the information requested, please enclose copies (not originals) of two of the following documents:

• One photo ID (e.g. passport, driving licence)
• One proof of address (e.g. utility bill, bank statement within last 3 months)
