What's Involved in an ADHD Assessment?
Before the Appointment
Before the appointment, you’ll receive some forms to fill out. These will involve questions about concentration, organisation, restlessness, impulsivity and daily functioning, to help the psychiatrist better understand you. Somebody close to you may also be asked to complete a questionnaire.
Remember: these forms support the assessment but cannot diagnose ADHD by themselves.

At the ADHD assessment appointment
During the assessment, an ADHD specialist will ask for specific examples of difficulties such as:
losing or forgetting things
struggling to begin or complete tasks
poor time management and chronic lateness
becoming distracted during conversations or work
restlessness or feeling constantly “on the go”
interrupting, making impulsive decisions or overspending
becoming overwhelmed by routine responsibilities
They will explore how these experiences affect areas including work, education, relationships, finances, driving, self-care and household responsibilities. Symptoms generally need to cause difficulties in more than one part of your life, rather than occurring only at work or during a particularly stressful period.
Your childhood history
ADHD is a neurodevelopmental condition, so the clinician will look for evidence that relevant traits were present before the age of 12. They may ask about:
concentration and behaviour at school
homework and exam performance
lateness, forgetfulness and losing belongings
friendships and family relationships
restlessness, daydreaming or impulsive behaviour
whether you achieved well but needed unusual amounts of effort or support
Not having school reports does not necessarily prevent an assessment. The clinician can consider your recollections and information from relatives, although independent supporting information can be helpful.
Other conditions and explanations
The assessment should include a broader review of your mental and physical health. This is important because anxiety, depression, trauma, autism, sleep disorders, substance use, thyroid problems and other conditions can cause or worsen ADHD-like symptoms. It is also possible to have ADHD alongside another condition.
There is no blood test, brain scan or computer test that can diagnose ADHD on its own. Some services use attention tests such as QbTest, but NICE states that these should supplement a specialist’s clinical judgement rather than replacing it.
How long does an ADHD assessment take?
The format varies depending on the type of appointment you book. The main assessment might last around 90 minutes to three hours, or it may be divided into two or more appointments.
After the assessment, you should be told whether you:
meet the diagnostic criteria for ADHD
have some traits but not enough for a formal diagnosis
may need assessment for another or additional condition
need further information before a conclusion can be reached
You will usually receive a written report explaining the decision and recommendations. Where ADHD is diagnosed, options may include practical adjustments, psychological support, coaching-style strategies and medication. Medication is not automatically started at the diagnostic appointment; it normally involves a separate discussion and checks such as blood pressure, pulse, weight, medical history and sometimes further cardiac assessment.
To prepare, write down concrete examples from childhood and adulthood, gather any school reports or previous mental-health records you have, and make a list of medications, sleep issues, physical conditions and family medical history.
